S/he will execute a set of responsibilities to enable the success of our clinical model, which is core to Oak Street’s success.ResponsibilitiesSpecific responsibilities include:

  • Key focus on health outcomes and controlling medical costs in our patient population
  • Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)
  • Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelines
  • Reporting and analytics in support of our Care Teams (physicians, nurses, care managers)
  • Establishing and maintaining relationships with both internal and external stakeholders
  • Other duties, as assigned

  • 2+ years of experience with direct patient care
  • Strong MS Excel skills
  • US work authorization
