In this exciting role, you’ll ensure that every patient receives timely and top-notch care as they transition into home health services.What You’ll Be Doing:As a Referral Specialist, your days will be dynamic and impactful:

• Manage Patient Referrals: Handle daily intake operations, ensuring smooth transitions with compliance to all intake policies and regulatory requirements.
• Build Lasting Relationships: Cultivate strong relationships with referral sources to ensure seamless and efficient transitions.
• Coordinate Patient Care: Oversee the education, care plans, and schedules to guarantee that patients receive top-tier care from the get-go.
• Maximize Reimbursement: Verify insurance, secure authorizations, and manage contracts to optimize reimbursement and ensure a smooth process for all parties.
• Community Resource Guide: Be a valuable resource for referral sources and patients, helping them connect to additional services as needed.

What You Bring to the Role:To thrive in this position, you’ll have:

• 3-5 years of experience in home health or hospice is required.
• Strong communication and public relations skills to build rapport with everyone you work with.
• A proactive, organized, and adaptable mindset—you take initiative and stay on top of every detail!
• Bilingual skills (preferred but not required).
