This is an excellent opportunity to make a meaningful impact in the lives of others, we look forward to meeting you!DESCRIPTION OF WORKAs an Aging Care Manager 2, you will provide services from the point of initial contact with older adults, their families, and other professionals through ongoing care management responsibilities. Some specific duties include maintaining a caseload of consumers and providing assistance with protective services, care transitions, and person-centered counseling. This position also involves regular coordination of services with physicians, community agencies, financial institutions, and more. Our ideal candidate must have the ability to manage multiple tasks and demonstrate close attention to detail, which includes completion of paperwork, work assignments, and meeting deadlines. As abilities and understanding of available programs progress, this professional will be assigned increased responsibility for arranging consumer services.Work Schedule and Additional Information:

• Full-time employment
• Work hours are 8:00 AM to 4:30 PM, Monday – Friday, with a 60-minute lunch.
• On-call hours when required.
• Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONSMinimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.
