Some specific duties include maintaining a caseload of consumers and providing assistance with protective services, care transitions, and person-centered counseling. This position also involves regular coordination of services with physicians, community agencies, financial institutions, and more. Our ideal candidate must have the ability to manage multiple tasks and demonstrate close attention to detail, which includes completion of paperwork, work assignments, and meeting deadlines. As abilities and understanding of available programs progress, this professional will be assigned increased responsibility for arranging consumer services.Work Schedule and Additional Information:
