Our team is growing and looking for a dedicatedPhysical Therapist (PT)Shifts Available: Full-Time & PRNService Areas with Openings: Northeast / Northwest / Southeast / Woodlands / Clear LakeIMMEDIATE NEED: Northeast Houston (Full-Time & PRN)Responsibilities:

• Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
• Implement physical therapy care plans.
• Coordinate care with the home healthcare team.
• Educate patients and family members about health conditions.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:

• Physical Therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
